Palliative Care - Home Support Service

(Bristol & South Gloucestershire)

What is the service?
The NHS Palliative Care Home Support Service (PCHS) provides personal care and emotional support for patients who wish to be at home in their final days or weeks of life.

Who is this service for?
Patients referred to the palliative care home support service must fulfil the following criteria:

• Be 18 years of age or older.
• Be assessed as being in the final 4-6 weeks of life or less.
• Be registered with a GP in the Bristol or South Gloucestershire area.

The service provides care regardless of patient condition. Referrals can be made by any health or social care professional.

Referral process
Once the patient has been assessed as being in their last 4 to 6 weeks of life and needs personnel care and emotional support, the referrer should download the referral form and fax it directly to 0117 982 1458.
