Introduction

The Palliative Care Home Support Service (PCHS) provides personal care and emotional support for patients who wish to be at home during their last four to six weeks of life.

The Cancer Help for minority Ethnic Communities Service (CHEC) aims to improve cancer care for black and other ethnic minority groups.

Both services are based at Avonmouth Medical Centre

Service Areas
Patients GPs must be based within the service area to be eligible for referral.

North Somerset including Portishead, Clevedon, Weston Super Mare, W-S-M, Nailsea, Long Ashton, Backwell,  Yatton,
S Glos, South Glos, South Gloucestershire including Thornbury, Downend Frenchay Warmley Wick Yate, Filton, Bradley Stoke, Patchway, Longwell green, Emersons Green Kingswood, Willsbridge
and all areas of Bristol including
