The Service Design Programme
The Bristol Health Services Plan Service Design team is transforming health services for the population of Bristol, North Somerset and South Gloucestershire.
The vision of future services in the region is to design and deliver high quality, safe and effective health services based on the agreed clinical strategy, known as the Bristol Health Services Plan Model of Care.

The objective of the Model of Care is to deliver a new health system characterised by:
- A strengthened primary care system
- Integrated rehabilitation services
- A range of specialist teams combining hospital and community expertise
- A comprehensive urgent care network
- An emergency/acute assessment and treatment service
- A strengthened critical care team
- Flexible inpatient services
- A systematic planned surgical service
- A community based health and social Children’s service
- Rapid response diagnostic services
- Responsive support
Implementing this clinical strategy will involve designing
changes to the way health care is currently provided. The
Service Design team aims to design new health services that:
- Respond to the health needs of local populations
- Improve health and health outcomes
- Provide personalised and accessible care
- Are patient focused
- Provide a positive experience
- Establish integrated care pathways
The end result will be better, quicker, more convenient services, and an improved patient experience.
