Rehabilitation Service Design and Development

The Rehabilitation Service Development Group is made up of a range of stakeholders from the following groups:

The group has a range of objectives to improve rehabilitation services throughout the Bristol region. These include developing a plan that:

develops a clear model for the division and interface between acute and community based rehabilitation.
provides a forecast of current and future needs for community based rehabilitation and ensure there are robust.
plans to deliver post hospital rehabilitation.
develops principles for community based rehabilitation to encompass: entry criteria; care pathways for rehabilitation (including both inpatient and outpatient models) and links to any specialists in the acute trust; consideration of workforce development issues.
ensures that rehabilitation includes and co-ordinates initiatives between PCTs and Local Authorities
