Rehabilitation Service Design and Development
The Rehabilitation Service Development Group is made up of a range of stakeholders from the
following groups:
- Clinical representation: primary and secondary care
- Commissioning representation (local NHS Trusts)
- Managerial representation: primary and secondary care
- Public Health
- Patient/user representation
- Voluntary and community organisations
- Adult Community Care Departments
The group has a range of objectives to improve rehabilitation services throughout the Bristol region. These include developing a plan that:
- develops a clear model for the division and interface between acute and community based rehabilitation.
- provides a forecast of current and future needs for community based rehabilitation and ensure there are robust.
- plans to deliver post hospital rehabilitation.
- develops principles for community based rehabilitation to encompass: entry criteria; care pathways for rehabilitation (including both inpatient and outpatient models) and links to any specialists in the acute trust; consideration of workforce development issues.
- ensures that rehabilitation includes and co-ordinates initiatives between PCTs and Local Authorities .
