End of Life Service Design and Development
The End of Life Service Development Group is made up of a range of stakeholders from the following groups:
- Clinical representation: primary, secondary and hospice care
- Commissioning representation (local NHS Trusts)
- Managerial representation: primary, secondary and hospice care
- Specialist Palliative Care
- Local Authorities
- Care Homes
- Public Health
- Users and Carers
- Voluntary and community organisations
The group has a range of objectives to improve end of life services throughout the Bristol region. These include developing a plan that:
- agrees on, implements and monitors progress against a vision for an integrated model of care for patients at the end of their life.
- improves health outcomes and patient experience for people in the last year of life through the development of responsive, effective and quality services.
- develop services that support patients to, where appropriate, die in the way they choose, in the place they choose.
- develop and implement a strategy for reducing emergency admissions from nursing homes.
- use the combined skills of primary care and local experts in specialist palliative care to the greatest advantage for patients and carers.
- Ensure that professionals feel able, qualified and supported to identify patients at the end of life and communicate with them about their wishes.
- Where necessary, organise the development of new services to the point of developing service specifications.
- Build on the accessibility and coordination of care and information across health and social care within the community, secondary care and voluntary sector organisations and specifically develop a model of access for 24/7 support and services for people in the last year of life.
- Develop a strategies for patient and carer involvement including, where appropriate, Equality Impact Assessments and recognising local gaps in life expectancy;
- Ensure service design is focussed on all patients who might need to access services, regardless of their age, diagnosis, where they live, what language they speak or where their GP practice is.
